Clomipramine Hydrochloride<\/p>\n
Anafranil is prescribed for the treatment of obsessive-compulsive disorder (OCD). It is also used in treating panic disorder, major depressive disorder, and chronic pain.<\/p>\n
The initial adult dose for OCD is typically 25 mg per day, which may be increased gradually. The maintenance dose usually ranges between 100 mg to 250 mg per day, taken in divided doses.<\/p>\n
In elderly patients or those with renal or hepatic impairment, the dosage may require adjustment. Regular monitoring and dose titration are advised.<\/p>\n
Clomipramine works by inhibiting the reuptake of serotonin and norepinephrine<\/a>, thereby increasing their levels in the brain and helping to stabilize mood and reduce anxiety.<\/p>\n Side effects include dry mouth, constipation, weight gain, dizziness, and blurred vision. These are generally mild to moderate in intensity.<\/p>\n Serious side effects include arrhythmias, seizures, and severe hypotension. These require immediate medical attention.<\/p>\n Anafranil interacts with monoamine oxidase inhibitors (MAOIs), selective serotonin reuptake<\/a> inhibitors (SSRIs<\/a>), and anticoagulants. Caution is advised when co-prescribing these medications.<\/p>\n Concurrent use of alcohol and Anafranil may exacerbate drowsiness and dizziness. Patients are generally advised to limit or avoid alcohol.<\/p>\n Contraindications include recent myocardial infarction, acute-phase recovery from coronary artery disease, and known hypersensitivity to clomipramine or any component of its formulation.<\/p>\n Clomipramine should be used in pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in breast milk, and caution is advised when administering to breastfeeding mothers.<\/p>\n In case of overdose, immediate medical attention is required. Symptoms of overdose include cardiac dysrhythmias, severe hypotension, and convulsions.<\/p>\n Anafranil should be stored at room temperature, away from moisture and direct sunlight. Keep it out of reach of children.<\/p>\n Regular monitoring of blood pressure, heart rate, and plasma levels is recommended. Periodic assessments of liver and kidney function are also advised.<\/p>\n Patients should be advised not to abruptly discontinue Anafranil due to the potential for withdrawal symptoms such as nausea, headache, and malaise.<\/p>\n Clomipramine is well-absorbed following oral administration, with peak plasma levels achieved within 2-8 hours. It has a half-life of approximately 32 hours.<\/p>\n Clomipramine is metabolized in the liver via demethylation and hydroxylation. The primary metabolite, desmethylclomipramine, is also active.<\/p>\n Anafranil is primarily excreted via the kidneys, with approximately 50% of a dose excreted in the urine and 24% via feces as unchanged drug or metabolites.<\/p>\n Patients should be informed about the onset of therapeutic effects, which may take 2 to 3 weeks.
