Lisinopril is not metabolized and is excreted unchanged primarily in urine.

Recommended Dosage

Hypertension: Starting dose is 10 mg once daily, with maintenance doses ranging from 20 to 40 mg daily. Heart failure: Starting dose is 5 mg once daily, with a maintenance dose from 20 to 40 mg daily. Post-myocardial infarction: Starting dose is 5 mg once daily, followed by 10 mg daily for six weeks.

Initial Monitoring

Monitor blood pressure, renal function (serum creatinine), and serum potassium levels before and during treatment.

Adverse Effects

Common side effects include cough, dizziness, headache, fatigue, and nausea. Serious side effects can include angioedema, hyperkalemia, severe hypotension, and renal impairment.

Drug Interactions

Concomitant use of potassium supplements or potassium-sparing diuretics may increase the risk of hyperkalemia. Use with diuretics may cause excessive reduction in blood pressure. Nonsteroidal anti-inflammatory drugs (NSAIDs) may reduce the antihypertensive efficacy of lisinopril.

Special Populations

Use caution in patients with renal impairment; dosage adjustment may be necessary. Not recommended for use during pregnancy due to potential risk to the fetus. In lactating women, the effect on breastfed infants is unknown; use with caution.

Contraindications

Contraindicated in patients with a history of angioedema related to previous ACE inhibitor therapy, hypersensitivity to lisinopril or any component of the formulation, and hereditary or idiopathic angioedema.

Storage Conditions

Store at controlled room temperature, between 15°C to 30°C (59°F to 86°F). Protect from moisture and light.

Patient Counseling Points

Inform patients about the potential for dizziness, especially when starting therapy. Advise them to avoid activities requiring mental alertness until they know how the medication affects them. Teach patients to report any signs of swelling, especially in the face or throat, immediately. Inform them to maintain adequate hydration and inform their healthcare provider of other medications or supplements they are taking.

Renal Protection

May provide protective effects on the kidneys in patients with diabetes and hypertension by reducing glomerular hypertension and proteinuria.

Alternative Treatments

Include other antihypertensive agents such as ARBs (angiotensin II receptor blockers), calcium channel blockers, and thiazide diuretics.

Generic Availability

Available as a generic medication, which may be more cost-effective for patients compared to the brand-name equivalent.

Impact on Elderly

In elderly patients, increased sensitivity to the blood pressure-lowering effects may be observed, necessitating careful dosage adjustments and monitoring.

Pediatric Use

Approved for use in pediatric patients 6 years and older for hypertension, but safety and efficacy in children under 6 have not been established.

Brand Name

Prinivil is the brand name of lisinopril as marketed by Merck & Co.

Prinivil, also known as lisinopril, is a medication used to treat high blood pressure and heart failure.
