Dosage Range

The usual starting dose of Benicar for adults is 20 mg once daily. The dosage may be increased to a maximum of 40 mg daily, based on blood pressure response. No initial dosage adjustment is required for elderly patients.

Administration Instructions

Benicar can be taken with or without food. Swallow the tablet whole with a sufficient amount of water. It is recommended to take it at the same time each day to maintain an even level in your bloodstream.

Drug Interactions

Concomitant use of Benicar with NSAIDs may reduce its antihypertensive effect. Combining Benicar with potassium-sparing diuretics or potassium supplements can increase the risk of hyperkalemia. Monitor for increased serum potassium in such cases.

Pregnancy Category

Benicar is classified as FDA Pregnancy Category D. Drugs in this category can cause harm to a developing fetus and should not be used during pregnancy, especially during the second and third trimesters.

Impact on Renal Impairment

No initial dosage adjustment is required for patients with mild to moderate renal impairment. Limited data are available for patients with severe renal impairment. Caution is advised under such conditions.

Impact on Hepatic Impairment

In patients with moderate hepatic impairment, the usual recommended starting dose is 10 mg once daily. Use with caution with increased monitoring in severe hepatic impairment due to limited clinical experience.

Pediatric Use

Benicar is not recommended for use in children under the age of 1 year due to safety concerns. For pediatric patients aged 6 to 16 years, the typical initial dose is based on body weight, usually starting at 10 mg daily.

Geriatric Use

Clinical studies have not identified differences in responses between elderly and younger patients. However, greater sensitivity of some older individuals cannot be ruled out. Monitoring of renal function and potassium levels may be particularly advisable in elderly patients.

Common Side Effects

Common adverse reactions include dizziness, headache, and fatigue. These effects are usually mild and transient. Patients should be advised to inform their healthcare provider if these symptoms persist.

Severe Side Effects

Serious adverse reactions may include symptomatic hypotension, especially in patients who are volume- or salt-depleted. Rare cases of renal impairment or failure have also been reported. Prompt assessment and appropriate management are recommended if severe side effects occur.

Clinical Trials Data

Benicar’s efficacy in lowering blood pressure has been assessed in several double-blind, randomized, placebo-controlled studies. These studies demonstrated significant reductions in systolic and diastolic blood pressure within 2 weeks of initiation. The full therapeutic effect is typically achieved by 8 weeks.

Overdose Management

In the event of an overdose, supportive therapy is recommended. Symptoms may include hypotension and tachycardia; bradycardia could also occur. Hemodialysis is not effective in removing olmesartan from the plasma.

Store Benicar tablets at room temperature, between 20°C and 25°C (68°F and 77°F). Keep the medication in its original packaging and protect from moisture. Ensure that it is kept out of reach of children.
