Medication Guide for Celebrex
Active Ingredient Information
Celebrex contains the active ingredient celecoxib. Celecoxib is a nonsteroidal anti-inflammatory drug (NSAID) that selectively inhibits cyclooxygenase-2 (COX-2), reducing inflammation and pain.
Intended Uses
Celebrex is prescribed for the treatment of various conditions including osteoarthritis, rheumatoid arthritis, ankylosing spondylitis, and juvenile rheumatoid arthritis. It is also indicated for the management of acute pain and primary dysmenorrhea.
Dosage Forms Available
Celebrex is available in capsule form. Commonly prescribed dosages include 50 mg, 100 mg, 200 mg, and 400 mg capsules. The capsule should be swallowed whole with water, and not crushed or chewed.
Mechanism of Action
Celecoxib works by inhibiting the COX-2 enzyme, which is responsible for the synthesis of prostaglandins. By reducing prostaglandin production, celecoxib effectively lowers inflammation and relieves pain.
Recommended Dosage
The typical dose for osteoarthritis is 200 mg per day, which can be taken as a single dose or as 100 mg twice daily. For rheumatoid arthritis, the dosage is usually 100 mg to 200 mg twice daily. Always follow the prescribing healthcare provider’s instructions.
Administration Guidelines
Take Celebrex with or without food. However, taking it with food or milk can help minimize gastrointestinal discomfort. It is important to maintain consistent timing when taking the medication to ensure stable blood levels.
Storage Requirements
Store Celebrex at room temperature, between 20°C to 25°C (68°F to 77°F). Protect it from moisture and light. Keep the medication in its original container and out of reach of children and pets.
Potential Side Effects
Common side effects include gastrointestinal issues such as diarrhea, indigestion, and abdominal pain. Other potential side effects include headache, dizziness, and upper respiratory tract infections. Report any persistent or severe side effects to your healthcare provider.
Drug Interactions Information
Celebrex may interact with other medications, including anticoagulants such as warfarin, other NSAIDs, ACE inhibitors, and diuretics. These interactions can increase the risk of side effects or reduce the effectiveness of either medication. Always inform your healthcare provider of all medications and supplements you are taking.
Special Populations Considerations
Celebrex should be used with caution in elderly patients, as they are at a higher risk of adverse reactions. Patients with liver or kidney impairment should also use Celebrex with caution, as dose adjustments may be necessary. Pediatric use is approved for juvenile rheumatoid arthritis in patients aged 2 years and older.
Monitoring Parameters
Regular monitoring of renal and liver function is recommended during long-term use. Blood pressure should be monitored periodically, as NSAIDs including celecoxib can cause new hypertension or worsen pre-existing hypertension.
Signs of Overdose
Symptoms of overdose may include nausea, vomiting, abdominal pain, lethargy, drowsiness, and gastrointestinal bleeding. In case of suspected overdose, immediate medical attention is necessary.
Non-Clinical Toxicology
Celecoxib has been shown to cause congenital malformations in animals at doses higher than those used in humans. It is classified as a Category C medication during pregnancy, meaning risk to the fetus cannot be ruled out.
Contraindications
Celebrex is contraindicated in patients with known hypersensitivity to celecoxib, sulfonamides, aspirin, and other NSAIDs, and in those with a history of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs. It is also contraindicated for peri-operative pain in the setting of coronary artery bypass graft (CABG) surgery.
Pharmacokinetics
After oral administration, celecoxib is well-absorbed with peak plasma concentrations reached within 3 hours. The plasma half-life is approximately 11 hours. Celecoxib is metabolized in the liver by the cytochrome P450 enzyme system, primarily CYP2C9, and is excreted mainly through feces.
Clinical Efficacy
Clinical trials have demonstrated the effectiveness of Celebrex in relieving pain and inflammation associated with arthritis and musculoskeletal conditions. It has shown favorable outcomes in reducing pain scores, improving joint function, and enhancing the quality of life in patients with chronic pain conditions.
Instructions for Missed Dose
If a dose is missed, take it as soon as you remember. If it is almost time for the next dose, skip the missed dose and resume the regular dosing schedule. Do not take extra doses to make up for the missed dose.
Handling and Disposal
Properly dispose of unused or expired Celebrex through take-back programs or based on guidelines provided by your pharmacy. Do not flush the medication down the toilet or pour it into drains unless instructed to do so.
Patient Counseling Information
Patients should be advised to immediately report any signs of gastrointestinal bleeding, such as black stools or vomiting blood. Inform them about the importance of periodic blood pressure and renal function monitoring. Discuss the risks and benefits of long-term NSAID use with your healthcare provider.
