Cefadroxil is a first-generation cephalosporin antibiotic. It is commonly used to treat various bacterial infections. Structurally, it is related to penicillins but has a broader spectrum of activity.

Mechanism of Action

Cefadroxil works by inhibiting cell wall synthesis. This bactericidal action is achieved by binding to penicillin-binding proteins, leading to cell lysis and death. The drug’s effectiveness relies on its ability to maintain a high drug concentration for an extended period.

Pharmacokinetic Profile

After oral administration, Cefadroxil is well absorbed. Its bioavailability ranges between 90% and 95%. It achieves peak plasma concentrations within 1 to 2 hours. Cefadroxil is widely distributed in body fluids and tissues. It is minimally metabolized in the liver and is excreted primarily via the kidneys.

Indications and Usage

Cefadroxil is indicated for the treatment of various bacterial infections including urinary tract infections, skin and soft tissue infections, and pharyngitis. It can also be used for bone infections and acute otitis media. The drug is effective against pathogens such as Staphylococcus aureus, Streptococcus pyogenes, and E. coli.

Dosage and Administration

The typical adult dosage of Cefadroxil ranges from 1 to 2 grams per day, divided into single or multiple doses. For pediatric patients, the dosage is calculated based on body weight, usually 30 mg/kg/day in divided doses. It can be taken with or without food, but taking it with food may reduce gastrointestinal discomfort.

Contraindications

Cefadroxil is contraindicated in patients with known hypersensitivity to cephalosporins or any of its components. It should also be avoided in patients with a history of severe allergic reactions to penicillins, due to possible cross-sensitivity.

Potential Drug Interactions

Probenecid can increase Cefadroxil levels by decreasing renal excretion. Nephrotoxic drugs like aminoglycosides and loop diuretics may increase the risk of nephrotoxicity. Oral anticoagulants may have their effects enhanced when used concurrently with Cefadroxil.

Adverse Reactions

The most commonly reported side effects include gastrointestinal disturbances like nausea, vomiting, and diarrhea. Hypersensitivity reactions such as rash, urticaria, and pruritus may occur. Rarely, more severe reactions like anaphylaxis, Stevens-Johnson syndrome, or toxic epidermal necrolysis may develop.

Precautions and Monitoring

Renal function should be monitored in patients with kidney impairment, and dosage adjustments may be necessary. Patients with a history of gastrointestinal disease, particularly colitis, should be monitored due to the risk of antibiotic-associated colitis. Hematologic parameters should be observed during prolonged therapy.

Patient Information

Inform patients to take the entire prescribed course, even if symptoms improve early, to prevent antibiotic resistance. Advise them to report any signs of an allergic reaction or severe adverse effect immediately. Encourage adequate hydration to aid in the elimination of the drug.

Special Populations

In pregnant women, Cefadroxil should only be used if clearly needed and after a risk-benefit analysis. It is excreted in breast milk in small amounts; thus, caution is advised when administered to nursing mothers. Dose adjustment may be required for elderly patients due to the usual decline in renal function with age.

Overdose Management

Symptoms of overdose may include nausea, vomiting, diarrhea, and gastrointestinal disturbances. Severe cases might present with neuromuscular excitability and seizures. Treatment is symptomatic and supportive. Hemodialysis can aid in drug removal in overdose situations.
