Mechanism of Action
Arimidex (anastrozole) is an aromatase inhibitor. It works by reducing the production of estrogen in the body. By inhibiting the enzyme aromatase, it decreases the level of circulating estrogen, which is important for certain types of breast cancer that rely on estrogen to grow.
Indications
Arimidex is primarily indicated for postmenopausal women. It is used to treat hormone receptor-positive early breast cancer. Additionally, it is prescribed for advanced or metastatic breast cancer after failure of tamoxifen therapy. It can also be utilized for first-line treatment of hormone receptor-positive or hormone receptor-unknown locally advanced or metastatic breast cancer.
Dosage and Administration
The recommended dosage is one tablet daily. It can be administered with or without food. Consistency in the timing of dosage helps maintain effective drug levels in the body. Tablets should be swallowed whole with water, not crushed or chewed.
Pharmacokinetics
Arimidex is rapidly absorbed with peak plasma concentrations reached within two hours. It has a half-life of approximately 50 hours, allowing for once-daily dosing. It is metabolized primarily by the liver and eliminated in urine and feces.
Drug Interactions
Co-administration with estrogen-containing therapies is counterproductive and should be avoided. CYP450 enzyme inducers or inhibitors may affect the metabolism of Arimidex, necessitating dosage adjustments or additional monitoring.
Side Effects
Common side effects include hot flashes, arthralgia, and nausea. Other possible side effects are fatigue, osteoporosis, and lower bone mineral density. Uncommon effects include peripheral edema and mood alterations.
Special Populations
Not recommended for premenopausal women. Its safety and efficacy in pediatric patients have not been established. Caution is advised in patients with severe hepatic or renal impairment due to altered drug clearance.
Storage Conditions
Store at a temperature between 20°C to 25°C (68°F to 77°F). Protect from light and moisture. Keep the medication in its original packaging until use.
Monitoring Parameters
Regular monitoring of bone mineral density is recommended due to the risk of osteoporosis. Liver function tests should be periodically evaluated. Serum cholesterol levels should also be monitored as an increase may occur.
Contraindications
Contraindicated in patients with known hypersensitivity to anastrozole or any inactive ingredients. Also contraindicated in pregnant or lactating women due to risk of fetal harm and insufficient data on milk excretion.
Patient Counseling
Patients should be informed about the purpose and expected benefits of the therapy. Educate on possible side effects and the importance of regular follow-ups. Emphasize adherence to the prescribed regimen for optimal outcomes.
Missed Dose
If a dose is missed, take it as soon as remembered unless it is almost time for the next dose. Do not double the dose to make up for the forgotten one.
Pharmaceutical Form
Arimidex is available in tablet form. Each tablet contains 1 mg of anastrozole. The tablets are white, film-coated, and round.
Disposal
Proper disposal of unused or expired medication is essential. Do not dispose of via wastewater or household waste. Utilize a take-back program if available.
Mechanism of Resistance
Some breast cancer cells develop resistance through mutations in the estrogen receptor. Another mechanism involves upregulation of alternate growth factor pathways, decreasing drug efficacy.
Clinical Trials
Clinical trials have demonstrated superior efficacy over tamoxifen in postmenopausal women. Studies have shown improved disease-free survival rates and delay in disease progression.
