Zovirax (Acyclovir) is an antiviral medication primarily used to treat infections caused by the herpes virus. Zovirax works by inhibiting the DNA synthesis necessary for the virus to replicate. Standard dosage forms of Zovirax include 200 mg and 800 mg tablets, 200 mg capsules, and 200 mg/5ml oral suspension. Topical forms are also available, including 5% cream and 5% ointment formulations. The specific form and dosage depend on the type and severity of the viral infection.
Zovirax tablets and capsules should be taken with a full glass of water. For the oral suspension, the dose should be measured with a dosing syringe or a special dose-measuring spoon or medicine cup. Topical forms should be applied directly to the affected area as directed by a healthcare provider.
For treating genital herpes, the initial dose is usually 200 mg taken five times a day for ten days. For recurrent outbreaks, a lower dose or a shorter duration of treatment may be prescribed. For shingles, the recommended dose is 800 mg five times daily for 7 to 10 days. The appropriate dose for children with chickenpox is based on weight, typically 20 mg per kg taken four times daily for five days.
Elderly patients may require a lower dose due to potential kidney function decline. Children’s doses are weight-based. In patients with renal impairment, dose adjustments are necessary to prevent medication accumulation and toxicity.
Zovirax inhibits viral DNA polymerase, preventing the synthesis of viral DNA. It is a nucleoside analogue that mimics the natural substrate of the enzyme, allowing it to be incorporated into viral DNA chains but terminating elongation. This prevents the virus from replicating and spreading.
After oral administration, Zovirax is moderately absorbed with a bioavailability of about 10-20%. It reaches peak plasma concentrations within 1.5 to 2 hours. Acyclovir is widely distributed in the body, including the cerebrospinal fluid. It is primarily excreted by the kidneys. The half-life of Zovirax in patients with normal renal function is approximately 2.5 to 3 hours.
Patients should be monitored for signs of renal and hepatic impairment, especially if long-term treatment is necessary. Periodic blood tests may be recommended to assess kidney function and blood cell counts. Any unusual symptoms such as persistent headaches, mental/mood changes, or signs of an allergic reaction should be reported to healthcare providers immediately.
Zovirax can interact with other medications. Probenecid and cimetidine can reduce the renal clearance of acyclovir, leading to higher plasma levels and increased risk of toxicity. Concomitant use with nephrotoxic drugs like amphotericin B and certain antibiotics (e.g., aminoglycosides) increases the risk of kidney damage.
Store Zovirax tablets, capsules, and oral suspension at room temperature away from light and moisture. Do not freeze the oral suspension. The topical cream and ointment should also be stored at room temperature, and the tube should be tightly closed when not in use.
If a dose of Zovirax is missed, take it as soon as remembered. If it is near the time for the next dose, skip the missed dose and resume the regular dosing schedule. Do not double the dose to catch up.
Common side effects of Zovirax include nausea, diarrhea, and headache. More serious but less common side effects include kidney problems, seizures, and thrombocytopenia. Topical forms may cause burning or stinging at the application site.
Zovirax should not be used in patients with known hypersensitivity to acyclovir or any of the formulation’s components. Caution is advised in patients with renal impairment or dehydration.
Zovirax should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is categorized as Pregnancy Category B, indicating that no well-controlled studies have been performed in pregnant women. The drug passes into breast milk, and caution is advised for nursing mothers.
Patients should be advised to drink plenty of water to stay well-hydrated during treatment. Emphasize the importance of adhering strictly to the prescribed dosage regimen and completing the full course of treatment. Patients should also be informed to avoid sexual contact when symptoms are present to prevent spreading genital herpes.
Resistance to Zovirax can occur through mutations in the viral thymidine kinase or DNA polymerase genes. Patients failing to respond to therapy or experiencing frequent recurrences should be evaluated for resistance. Sensitivity testing might be performed in complex cases, using cell culture assays to determine the effectiveness of Zovirax against the specific viral strain.
