Generic Name

Pantoprazole sodium.

Drug Class

Proton pump inhibitor (PPI).

Formulations Available

Delayed-release tablets (20mg and 40mg) and oral suspension.

Mechanism of Action

Pantoprazole inhibits the H+/K+ ATPase enzyme system at the secretory surface of gastric parietal cells, blocking the final step in acid production.

Primary Uses

Protonix is prescribed for the treatment of gastroesophageal reflux disease (GERD), erosive esophagitis, and pathological hypersecretory conditions including Zollinger-Ellison syndrome.

Administration Guidelines

Administer 40mg orally once daily, at least 30 minutes before a meal. For the oral suspension, mix with apple juice or applesauce, consume immediately.

Duration of Therapy

Treatment duration varies but typically ranges from 4 to 8 weeks. For maintenance therapy, longer durations may be prescribed.

Contraindications

Protonix is contraindicated in patients with known hypersensitivity to any component of the formulation or to substituted benzimidazoles.

Adverse Reactions

Common side effects include headache, diarrhea, nausea, abdominal pain, vomiting, flatulence, dizziness, and arthralgia. Serious adverse reactions include Clostridium difficile-associated diarrhea and vitamin B12 deficiency with long-term use.

Drug Interactions

Pantoprazole may interfere with drugs requiring an acidic pH for absorption. It may also interact with warfarin, methotrexate, and antiretroviral drugs.

Monitoring Requirements

Monitor for relief of symptoms, adverse reactions, and lab tests like liver enzymes, particularly in prolonged treatment durations. Periodic assessment of renal function is advisable.

Special Populations

In patients with severe liver impairment, dose reduction may be necessary. In elderly patients, cautious use is advised due to a higher frequency of side effects. Pediatric use for short-term treatment of GERD and erosive esophagitis is recommended in children aged 5 years and older.

Storage Instructions

Store delayed-release tablets and oral suspension packets at room temperature, between 20°C to 25°C (68°F to 77°F). Protect from moisture and light.

Overdosage Information

In case of overdose, symptoms may include confusion, drowsiness, blurred vision, tachycardia, nausea, vomiting, and sweating. Treatment is symptomatic and supportive.

Pharmacokinetics

Pantoprazole is absorbed after oral administration with peak plasma concentrations occurring approximately 2.5 hours post-dose. Its bioavailability is about 77%. It is primarily metabolized in the liver, with a half-life of about 1 hour.

Patient Counseling Points

Inform patients to take Protonix exactly as prescribed, preferably early in the morning before meals. Emphasize the importance of not splitting, crushing, or chewing the tablets. Advise patients to report severe, persistent side effects, and to inform healthcare providers about all concurrent medication usage, including over-the-counter drugs and supplements.
