Zocor

Zocor is a medication used to lower cholesterol and triglycerides in the blood. Its active ingredient is simvastatin. Alternative names for Zocor include Simlup, Simvacor, and Simvastatin. It helps reduce the risk of heart disease and stroke.

Indications for Use

Zocor is primarily indicated for the treatment of hypercholesterolemia and the prevention of cardiovascular disease. It helps in reducing low-density lipoprotein cholesterol (LDL-C), total cholesterol (Total-C), apolipoprotein B (ApoB), and triglycerides (TG) while raising high-density lipoprotein cholesterol (HDL-C) in patients with primary hyperlipidemia or mixed dyslipidemia.

The usual starting dose for Zocor is 10-20 mg once daily in the evening. For patients requiring a significant LDL-C reduction (more than 45%), a 20 to 40 mg starting dose may be prescribed. Adjustments should be made at intervals of no less than 4 weeks, up to a maximum of 80 mg daily.

Zocor should be taken in the evening, with or without food. This timing is crucial because cholesterol synthesis in the body is highest at night. Avoid the intake of grapefruit juice when taking Zocor as it can significantly increase the risk of side effects.

Simvastatin is absorbed orally and metabolized in the liver to its active form. The drug has a high first-pass effect, with approximately 5% reaching systemic circulation. Maximum plasma concentrations are achieved within 1.3 to 2.4 hours post-administration. The half-life varies based on the individual but is generally around 2 hours for the active metabolite.

Contraindications

Contraindicated in patients with active liver disease or unexplained persistent elevations in hepatic transaminase levels. Should not be used in patients who are pregnant or may become pregnant due to the potential for harm to the fetus. Breastfeeding is also contraindicated as it is unknown if Zocor is excreted into human milk.

Drug Interactions

Interaction with potent CYP3A4 inhibitors (e.g., itraconazole, ketoconazole, erythromycin, and protease inhibitors) significantly increases the risk of myopathy/rhabdomyolysis. The concurrent use of amiodarone, verapamil, or diltiazem also requires caution and dose adjustments as they may increase simvastatin concentration.

Common side effects include headache, nausea, vomiting, diarrhea, abdominal pain, and muscle cramps. Rare but severe side effects include myopathy and rhabdomyolysis, which are more significant at higher doses and with interacting medications.

Periodic lipid panel testing is essential to determine the efficacy of the treatment and need for dose adjustments. Liver function tests should be performed before starting therapy and thereafter as indicated by clinical symptoms suggestive of liver injury. Patients on higher doses should be monitored more closely for muscle pain, tenderness, or weakness.

Zocor should be stored at room temperature, between 59°F and 86°F (15°C and 30°C). It should remain in its original, tightly closed container and kept away from moisture. It is critical to ensure that the medicine is kept out of the reach of children.

Mechanism of Action

Zocor functions by competitively inhibiting the HMG-CoA reductase enzyme, which is imperative in the biosynthesis of cholesterol. The reduction in intracellular cholesterol levels leads to an upregulation of LDL receptors, increasing the clearance of circulatory LDL and decreasing plasma cholesterol levels.

Pharmacodynamics

Reduction of cholesterol levels is evident within 3-5 days of initiation, with maximum therapeutic response achieved in 4-6 weeks. The lipid-lowering effects are maintained during prolonged therapy.

Significant reductions in total cholesterol and LDL-C were observed in controlled trials. Patients experienced a mean decrease in LDL-C of 35-45% with doses ranging from 10 to 40 mg. In secondary prevention trials, simvastatin effectively reduced the risk of myocardial infarction and stroke by approximately 30%.

For elderly patients, starting at a lower dose under close monitoring is recommended, due to the increased risk of myopathy. Patients with renal impairment may require dosage adjustment and close monitoring for adverse effects.
